Overview

Below you will find information on how to file a cardholder dispute claim for issues with your EWF debit cards. Please submit ALL paperwork within 10 calendar days of opening your claim. After all paperwork is received, an investigation is initiated and adjudicated within 30-90 days. You will be notified of the decision along with any compelling evidence once the investigation is finalized.

NOTE: If you believe someone fraudulently submitted an application, or someone you know (like a household member or coworker) has fraudulently used your card, please contact your local police department or other authorities for assistance. Those claims generally will not be handled through the dispute process explained below.

  1. Visit nysewf.myprepaidcenter.com
  2. Sign into your Prepaid Card Account. 
  3. Review your transaction history,
  4. Download the Dispute Form and provide all applicable information along with the transactions you are disputing.
  5. Get the Dispute Form notarized by a Notary Public. 
  6. Bring the notarized Dispute Form into your local Police Department to initiate a police report. 
  7. Send your notarized Dispute Form and Police Report:

By Phone

  1. Contact the number on the back of the card, available 24 hours, 7 days a week.
  2. Cardholder Dispute Representative will intake your case and send you the required information to validate transactions and complete the Dispute Form.
  3. Get the Dispute Form notarized by a Notary Public, then bring it to your local Police Department to initiate a police report.
  4. Send your notarized Dispute Form and Police Report:

How Do I Find a Notary?

The following locations may be able to assist you with notarizing a document: 

 

  • Your auto/home or rental insurance agent
  • Banks and Credit Unions you belong to
  • Your real estate agent
  • Your County Clerk's office
  • Some colleges or universities you attend
  • Your public library
  • Mailbox Stores you have a mailbox with (may charge a small fee)
  • Family and friends who are licensed notaries
  • Some police departments
  • Your tax preparer
