Amusement Device Permitting in New York State

Management System for Protecting Workers Rights (MPWR)

Beginning in 2024, the New York State Department of Labor will be processing Amusement Device applications and supporting documentation through this new on-line process.  If you are a new or an established ride operator, please start the process by visiting https://dol.ny.gov/mpwr and creating or using your existing my.ny.gov acct account for access MPWR.

We believe that you will find many advantages with this new system such as:

  • Create and submit Amusement Device Applications.
  • Create and submit Location Notices.
  • Submit Proof of Workers Compensation, Disability, and Liability Insurance.
  • Pay online with a Credit Card.
  • Submit and upload additional documentation.
  • Receive real-time updates regarding the status of your application and/or issues which require your attention to complete the processing.

For your convenience, there are two training videos which will assist you with the process of sending your information to us for processing.

Submission Requirements

Amusement device operators operating in New York State must submit all of the following, annually, at least 10 days before the ride’s first use:

  1. Application for Amusement Device, Viewing Stand or Tent Operating Permit submitted through MPWR
  2. Proof of Liability, Workers’ Compensation and Disability Insurance (upload proof documents through the MPWR system).
  3. Proofs of Insurance
  4. • Certificate of Liability Insurance - One Million Dollar minimum occurrence based coverage and per occurrence coverage: (Acord 25 form or similar).
  5. • Certificate of Workers’ Compensation (see list of acceptable forms below)
  6. • Disability Insurance Coverage (see list of acceptable forms below)
  7. *Please Note: All insurance proof must list NYS DOL as certificate holder.
  8. Location Notice submitted through MPWR
  9. $100 application fee, per device paid online through MPWR

Adding a new location

If you need to add to your Location Notice during the season, you can do it through MPWR.

Adding a new amusement device

If you need to add another amusement device during the season, you must submit an additional Application for Amusement Device, Viewing Stand or Tent Operating Permit and $100 application fee, per device through MPWR.
